Upon receipt of a written request, the IHS Scholarship Program will verify your status and release pertinent information from your file for the purpose of obtaining credit (from a credit card company, bank, department store, etc.). Your request must include your signature and Social Security number. We cannot respond to credit inquiries by telephone.
